Re: Violation of ERISA by SAMBA. This date I sent the following message to the United States Office of Personnel Management, ERISA Compliance Division.      Intentional and malicious falsification of SAMBA records, cancellation of my dental & vision insurance coverage for the purpose of harassment. Possible criminal violation of ERISA by SAMBA anonymous employee. This date I confirmed by inquiry at SAMBA and at my bank (Chase) the following facts which SAMBA DOES NOT DISPUTE: I have been a SAMBA member in good standing for about half a century and I have never been late to pay the premiums. My SAMBA member ID: ............. 1)In January, 2015 I submitted and SAMBA received a change of bank for automatic payment (via debit of account) of my SAMBA premium for dental & vision care (D & V). 2) By letter dated  February 11, 2015 SAMBA notified me that my coverage for D & V  is cancelled. 3) *By my email and letter to SAMBA dated February  17 and 18, 2015, I notified SAMBA of their violation of ERISA. 4) SAMBA admits on February 18, 2015, that someone at SAMBA entered an incorrect bank account number by omitting one digit and thereby wrongly canceled my D & V coverage for non payment of premium.5) My bank confirms that if SAMBA enters the nine digit routing number and the nine digit account number, then SAMBA would be able to debit the account in the amount authorized; that the computer does not accept partial account numbers because such an entry would reflect no identifiable account.6)SAMBA confirms that someone removed one digit from the account number so as to nullify the transaction of debiting my account 7) SAMBA made no effort to check their own records to see their error. The SAMBA employee who can verify that such actions are suspicious is the honest and forthcoming Leah McKenzie. 8) I immediately (on 2-18-15)complied with SAMBA directive to request reinstatement of coverage for the D & V by sending via US Express mail, two money orders to SAMBA, each in the amount of $84.75 for February and March, 2015. I also sent another authorization form and canceled check to SAMBA for future automatic payments. I previously provided SAMBA with all appropriate documentation regarding a *change of bank for automatic payment of the monthly $84.75 premium; I also confirmed with a SAMBA representative that all documents were timely submitted to SAMBA.  Today, in efforts to conform to SAMBA directives, I forwarded via United States express mail to SAMBA 2 money orders in the amount of $84.75 each for February and March, 2015, premiums. I unsuccessfully tried to reach Ms. McKenzie by phone today to pay the premium and to otherwise remedy the problem.  I request that you kindly check your records to see that I timely submitted all necessary change of bank forms, that SAMBA provided me with no opportunity to correct the error which may have been caused by SAMBA, that SAMBA made no effort to contact me as required by SAMBA policy, that SAMBA blocks my efforts to talk with a SAMBA dental/vision representative.  Please reinstate coverage immediately and process my change of bank notification which are in your possession and which are also in the mail again now to SAMBA. I can if you permit make any additional payment necessary on the phone. I submit this request in accordance with ERISA.  I trust that the errors that caused the adverse action against me were simply clerical.
